What To Expect
Our goal is to provide exceptional healthcare to women of all ages in a comfortable and friendly atmosphere. To keep us on time, please arrive fifteen (15) minutes before your scheduled appointment time. We ask that you take a few minutes to complete our new patient forms in advance (in black ink only) and bring them with you for your appointment. Please do not return the forms in the mail. You will also need to bring your driver’s license (or other photo ID), your health insurance card, and a list of your medications. You will be asked to make any copayment required by your insurance when you check in for your first appointment.
Please familiarize yourself with your insurance policy as it is your responsibility to know if you will need a referral from your primary care physician. Please feel free to talk with any of our staff if you need assistance or have special needs. Suggestions are always welcome. We look forward to seeing you soon!
We do not allow food or drink in our office under any circumstances, and we ask that any accompanying family or friends use the restroom located in the main hallway just outside our reception area. It can sometimes be a little chilly, so feel free to bring a blanket if you like. If you must bring a child, please bring an adult to provide supervision during your appointment. This is for the protection of you and your child. We have equipment and computers in our exam rooms which can be a safety issue for children and a replacement cost issue for the patient. Thank you for your cooperation in this matter.
Disability Forms & FMLA Records
Please allow 7-10 days for processing of any medical records requests, disability or FMLA forms. We do not charge you for copies of your medical records. We do charge a fee for copies requested by attorneys, life insurance companies or disability companies. Due to staff time in completing disability and/or FMLA papers, we find we must charge a fee for this service. The fee for a “Simple Form” is $10.00. The fee for a “Complex Form” (2+ pages) is $25.00. This fee must be paid in advance of form completion. it will also be necessary for the patient to complete any patient areas and signatures prior to bringing the form to us. We will provide you with a statement documenting an appointment date and time, or a release to return to work after illness or surgery at no charge to you.
